Please complete and fax the one time set up submission form listed below under Important Forms. This allows your clinic to set up your submission in our system to avoid emailing issues. Electronic applications will only be accepted from the email address on file for clinics that have registered for this service.
Images must either be attached to an e-mail or mailed on a CD. We do not accept images from imaging services (Antech Imaging, View Cloud, etc) nor direct DICOM transfers.
CD’s MUST come from the submitting vet clinic. We will not accept CD’s from owners.

All digital images submitted must include dog identification, clinic name, and date of radiography overlaid on image. See detailed instructions for ID and positioning on second page of OFA hip/elbow application form.
The OFA application and payment should be included together along with the CD.
CDs may contain multiple images per dog, but please limit each CD to one dog.
Applications with images on CD will only be accepted from clinics that have registered for this service. Use the electronic submission registration form below. Please do not fax apps if mailing a CD
Dog id must be able to be seen regardless of the viewing software. If you can only see the dogs ID with the viewer on the CD then you will need to add a text box.
Can submit either jpg, jpeg or Dicom formats.

The OFA application may be scanned and included in the email along with image, or it may be faxed. If you are emailing the images the application must come at the same time as the email.
Please do not send us links through Antech or View Cloud, etc. images need to be attached to the email as an attachment.
Please list dog name in subject line of email.
Please include dog/owner info in the body of the email along with contact info for the clinic in case we have problems/questions or did not received something.
The fax number for sending applications is 573-875-5073. If sending the application via fax, please include a cover sheet indicating that the image will be digital and will be sent electronically. The email containing the image should include a note that the application was faxed.
Include credit card payment information in the appropriate section of the application form or on a separate sheet attached to the email or faxed to the OFA office (573-875-5073). Required credit card information includes: Cardholder Name, Card Number, Expiration Date, and 3 Digit Security Code. For more accurate billing if there are credit card issues, please indicate whether this is an owner payment or a veterinary clinic payment. If you would like to keep a credit card on file with the OFA office, please contact Carol Skinner at 573-442-0418 x221.
If multiple applications are submitted in a single email, please make sure each file is uniquely and appropriately named.
