OFA Health Clinic Submissions

The OFA will publicize your upcoming health clinic for free on the OFA website. To help us get your clinic listed accurately, please send your clinic details either in email or as a Word document in plain text (no fancy formatting). Please also use upper/lower case text, not all-caps.

Please submit the following to ofa@offa.org:

• Date(s) of Clinic

• City and State/Province where the clinic will be held

• Full address of location of clinic, including zip code

• Name of Sponsoring club/organization

• Time(s)

• Clinic tests offered including veterinarian performing test and fees.
  For cardiac, please indicate if auscultation, echocardiogram, Doppler, etc.
  Indicate if sedation will be offered for hip/elbow tests
  If any tests are breed specific, please note

• Whether pre-registration is required and if so, the date by which people must sign up.

• Contact information including email and/or website

A supporting pdf document with registration form can be attached. Please do not send a *.jpg file.
